Patient Portal

The portal contains the office policies forms and a long list of helpful patient educational handouts. You can access via My Health tab on left-hand side, and along the top, locate the Medical Forms tab.

Consent Forms

Consent forms will be sent to you four days in advance of your appointment via text or email to complete your check in process. They are listed below if you would like to read them before that time.

Medical Records Release Form

These forms allow us to request medical records from another provider or to send your records to the provider stated on the form. Please download and return back to us either by email, fax, or in person.

Insurance Coverage Inquiry

We advise you to check your insurance plan coverage before your initial appointment. Please use this form and call the number on the back of your insurance card or go online to the payer website to fill out the form with the recommended inquiries so you are fully informed regarding your financial obligations. Although we are contracted with many insurance carriers, we are not in network with all of their plans.
